For routine dental services, cleanings, or to set a time for a consultation for reconstructive or cosmetic dentistry, please use this form to request an appointment.

This service is for appoinment requests ONLY.  Appointment CANCELLATIONS ARE NOT ACCEPTED on this website.  Cancellation fees, if applicable, will apply to any appointment cancellation requests on this website.
